MEMBERSHIP TERMS AND CONDITIONS

I understand my membership is for regular installments via direct debit, and I am committing to pay this membership in full whether I am attending training or not.

I understand members train only as per the agreed number of times per week. Unused sessions do not roll over but recovery of session can be discussed directly with the director.

 

I understand camps will be held up to once every school holiday and I am entitled a 15% discount as member of DOC GK Academy using code DOCMEMBERSHIP

I understand that for the duration of the membership I’ll have access to a 15% discount all DOC Goalkeeping items using code DOCMEMBERSHIP

​

I understand that I can upgrade my membership at any time and this will be effective immediately. To downgrade my membership, the minimum term sessions must be completed and give 14 days notice.

I understand I am responsible for disclosing all relevant medical issues to DOC GK Academy in form of a notice from my medical professional

 

I understand I may apply to suspend my membership for up to 4 weeks of personal holidays and up to 6 weeks due to injury or illness, by supplying a medical certificate. Suspensions will not be accepted if there is an outstanding balance.

I understand that no membership credit is given for cancelled memberships, and no refunds are given for returning memberships or gears under any circumstances

I understand that Goalkeeper Training is a potentially dangerous activity that:

• there is a possibility that injuries may occur during your training

• students are responsible for their own safety and the safety of those around them

• students have the right and responsibility to excuse themselves from any exercise they believe could be harmful to them

• DOC GK Academy is not liable for any injuries incurred

Cancellation of membership requires a minimum of fourteen (14) days written notice after the minimum term of 3 months has been completed. This to be done by completing the appropriate form provided by DOC GK Academy . Weekly debit will continue to apply fourteen (14) days from this point. Notice to cancel a direct debit membership cannot be provided verbally or via telephone.

I GIVE  the Academy consent to use my image and likeness in its publications, advertising or other media (including the internet). This consent is given in perpetuity and without expectation of compensations or remuneration
